If you are planning to be accompanied by a rider in equipment owned and operated by Karl R. Johnson Trucking, Inc you are required to fill out this Hold-Harmless agreement and have it signed by a company official prior to having a rider with you . •  EXPENSE SHEETS

If you have bought any supplies, fuel, repairs, postage, tolls with your road money please submit your receipt with an updated Expense Sheet. If you do not have an expense sheet please click the icon below to download the form you will need to be sure that you are reimbursed in your paycheck.

• REQUEST FOR TIME OFF SHEET

If you would like to request a date for vacation time or if you know in advance of a date you need to have off from work, you are required to submit a request for time off sheet to the office with you paperwork at least 30 days prior to the date you are requesting.
